More Small Business Health Insurance Basics In Texas

Because premiums, deductibles, copayments, and coinsurance levels for small business group health insurance policies in Texas can vary widely from plan to plan, it pays to shop around.

Have a good understanding of your employees’ healthcare needs before you start shopping. Do they require frequent medical care or do they rarely see the doctor? Are they more concerned about preventive checkups or coverage in case of emergency? Are prescription or maternity benefits important to them? This is an essential first step. You want to purchase a plan that offers the medical benefits your employees need, without a bunch of “extras” your employees won’t take advantage of. When shopping for coverage, the Texas Department of Insurance recommends keeping these guidelines in mind:

- Be sure you understand the full extent of each plan’s coverage when comparing plans and rates. If you decide to go with a consumer choice health benefit plan over one with all the state-mandated benefits, the carrier or agent is required to explain in writing which coverages you don’t have.

- Plans with higher deductibles, copayments, and employee share of coinsurance generally will have lower premiums. Keep in mind, however, that your employees will also have to pay more out of pocket when they access services or benefits.

- Consider factors other than cost, such as a company’s financial strength and complaint record. These are indicators of the service you can expect. You can learn a company’s financial rating, as determined by an independent rating organization, by calling the Texas Department of Insurance (TDI) Consumer Help Line. - Look into purchasing cooperatives. These are groups of small employers with similar health care needs who join together to negotiate discounted rates for shared plans. For a list of registered purchasing cooperatives in Texas, call the Consumer Help Line.

- Buy only from licensed insurance companies. Selling unlicensed coverage is illegal in Texas. If you buy from an unlicensed carrier, your employees’ claims could go unpaid and you could be held liable for the full amount of your employees’ claims and losses. Guaranty associations pay the claims of licensed carriers that become insolvent. You can learn whether a company is licensed by calling the Consumer Help Line.

- Understand that employee health coverage is different from workers’ compensation insurance, which covers only job-related injuries and illnesses. Although workers’ compensation insurance is not required in Texas, it protects you from high damage awards in the case of workplace accidents. Providing regular health coverage to your employees is not a legal alternative to providing workers’ compensation insurance.

Who Pays and How Much?

The law doesn’t require employers to contribute toward health benefit plan premiums. However, many carriers require employers to pay at least 50 percent of the plan’s premiums. Employers may choose to pay a higher percentage than the carrier requires.

The carrier must offer dependent coverage to all eligible employees. Generally, employers are not required to contribute toward the cost of dependent coverage. If the employer doesn’t contribute, employees may have to pay all of these costs themselves.

Premiums may increase at each renewal term, largely due to rising health care costs and possibly as a result of employee claims experience. Texas law caps small-employer rate increases due to health factors at 15 percent per year.

Insurers cannot require businesses to purchase additional lines of insurance, such as life insurance or disability insurance, as a condition of the sale of a health plan.

Employee Signup and Waiting Period

New employees must be given at least 31 days from their start date to enroll in a plan. After this time, they may be required to wait up to one year for the next “open enrollment period” to join. Carriers must offer a 31-day open enrollment period annually.

You can choose to require your employees who enroll in a plan to wait up to 90 days before being eligible for benefits. During this period, the carrier may not charge you or the employee a premium.

Carriers may require participants to wait a certain amount of time before covering pre-existing medical conditions. In general, plans have different rules for pre-existing conditions. Plans using the open-enrollment requirement cannot make new members wait more than one year before covering their pre-existing conditions.

New enrollees who were covered in the year prior to joining a plan also receive credit toward the waiting period on a month-for-month basis. For example, an employee who was covered under creditable coverage for the entire year before joining a new plan would receive 12 months credit toward a one-year pre-existing condition wait — and would therefore experience no wait at all. For previous coverage to be considered creditable, there may not have been more than a 63-day break between the end of the previous coverage and the start of the new coverage.

A small business employer carrier cannot refuse to provide health coverage for employees on the grounds of employee illnesses or pre-existing conditions. Nor may carriers use health-related factors — such as employees’ prior claims experience or information on conditions arising from violent family situations — to decide whether to provide coverage.

How Small Employer Plan Premiums are Calculated

The rates for any given small employer plan are not solely determined by the benefits and deductibles of the plan itself. Certain objective “case characteristics,” along with any health status-related factors of employees, may also be components in determining the premium rate for the small employer group. Case characteristics consist of age, gender, group size, industry, and geography. Carriers can use some or all of these five objective criteria:

- Age of employees: Older people can reasonably be expected to have more expensive and more frequent health-related claims. Generally, the older your workforce, the more your plan will cost.

- Gender: Females generally incur higher medical costs than males at younger ages, particularly during childbearing years. The variance diminishes with age until medical costs for males begin to exceed those for females as they near ages 50 and 60. If you have a younger, proportionately more female workforce, or one that is older and proportionately more male, expect to pay higher premiums.

- Number of plan participants: Carriers often base rates on group size for two reasons. As size increases, administrative costs per insured decrease. Also, smaller groups tend to buy health coverage based on the targeted needs of participants, increasing the likelihood of claims for the benefits provided. As group size increases, this “custom-tailoring” becomes more difficult and premiums tend to decrease. However, the highest group size factor may not exceed the lowest group size factor by more than 20 percent.

- Industry: Some industries have higher medical claims costs than others because of working conditions and the prevalence of accidents. High employee turnover in some industries can also result in higher administrative costs for the carrier. However, the highest industry factor a carrier charges may not exceed the lowest factor by more than 15 percent.

- Geographic area: Health care costs vary by region due to differences in cost of living and medical practices, as well as the amount of medical competition in the area. Most plans vary rates by either county or ZIP code, using the employer’s business address to set rates.

The rating process for a small-employer group can be described as a two-step process. First, a carrier determines a premium rate based on case characteristics and plan design, without regard to health status-related factors. This produces the baseline price of the policy. Second, the carrier may adjust the rate to reflect health status-related factors of the group. This adjustment must apply uniformly to all members of the group and may not exceed 67 percent of the baseline price of the policy.

Group health insurance can be not affordable for many small businesses, not to mention an administrative headache. Another alternative to group health insurance plans is to offer individual health insurance options to your employees. By law, an employer is not allowed to contribute to these plans, or that would be treated as group insurance under Texas state law. But you can still help your employees become insured in a good plan and improve their health and well-being and also improve employee retention in the process. Florida Health Insurance Minority Health Initiative

State Senator Arthenia Joyner (D-Tampa) on Tuesday welcomed reassurances from the governor’s office that a move to transfer the innovative Office of Minority Health into a sprawling division within the Florida Department of Health would be rejected.

The decision came less than 24-hours after the Tampa Democrat sent a letter to Governor Charlie Crist, urging him to rethink his agency’s pending action.

“The Office of Minority Health is currently on the cutting edge with regard to research, cultural competency and health equity policy,” Joyner wrote. “This momentum must be continued. Moving this office into a large department division will most certainly diminish the capability of the office to continue developing and implementing direly needed policies and strategies essential to eliminating health disparities among racial and ethnic populations.

“It is my deepest fear that the focus and progress of the program will be lost by swallowing this mission within an immense bureaucracy.”

At issue was an abrupt decision by DOH to transfer the highly effective Office of Minority Health into the agency’s sprawling Division of Family Health Services.

Established by the Legislature five years ago, the Office of Minority Health was launched by the former governor as a way to close the widening gap in health care for minority and ethnic residents by providing community medical outreach services, among other programs.

Since then, the group has made significant headway in rolling back not only the disparity in health services available to such groups, but the high costs taxpayers shoulder due to over-reliance on indigent emergency room care.

This has been accomplished at little cost to the state. Staffed by five including the director, only three full time members of the unit are paid by the state; the other two are funded through federal grant money.

“Since its inception, the Office of Minority Health has been extremely effective, already touching 50,000 lives throughout Florida and saving taxpayers potentially millions of dollars,” said Sen. Joyner. “The medical treatment this office spearheads costs on average about $100. Compare that to the typical emergency room visit costing taxpayers roughly $1600. That’s a huge savings.”

Cut this last legislative session by $1 million, the office has been struggling to complete a series of innovative initiatives, among them an agency-wide strategic and operational plan, along with a county-by-county database so that private health care organizations and residents can track community needs and progress.

The sudden decision to absorb into a larger, more cumbersome bureaucracy such a dynamic office noted as much for saving lives as for saving money made little sense, particularly in light of the economic conditions the state currently confronts, according to Joyner.

“Nationwide, our state comes in 45th in health care rankings, much of it due to the lack of health insurance and health care options for a large group of our diverse population. The worsening economy is only compounding matters.”

The Senator said she was heartened to hear that the governor pledged to protect the group’s independence, while continuing to report to the agency’s deputy surgeon general.

The human race walks a common road but once in awhile there is a revelation so profound that it changes the journey and all observations.

It is then not possible to return unaffected and compliant with old beliefs, but begin to embrace a vision of wholeness and balance in all of Nature’s cycles. It is a return to the bounty of what Nature has provided immemorially for total health; thereby, redressing our shopping habits,

dinner tables and dietary supplements.

This exercise is guaranteed to make you never look at a vegetable or fruit in the same way again. Nature’s plant foods are intrinsic to our body’s structure and function. As importantly, when we choose our supplements, it is Nature’s whole plants that best meet the needs of the internal pharmacist inside us.

A couple years ago I was walking in similar shoes as most people believing several commonalities. Clearly, there is a direct link between

the food we eat and the structure and function of our cells.

Clearly, it is wise to eat several servings of fruits and vegetables daily highly recommended by many organizations, but often not substantially followed. Commonly, we understand that modern agricultural and food manufacturing processes have depleted and denutriented our natural plants of many of the essential elements

Subsequently, we are informed to do the next best thing …SUPPLEMENT…SUPPLEMENT… SUPPLEMENT

Now there are whole volumes that deal with vitamins, their role in health, best sources and more. But without a lot of training, most of us go to a store and decide to buy but based on what criteria. And on store shelves we are accosted by every kind of fancy labeling, claims, pills, capsules, powders, potions …and for some of us, the cheaper the better buy, right?

And once in awhile there appears a new buzzword of something vital for your body, accredited by a doctor or two, and so we have to purchase that, too. Often, to make sure we were getting a full spectrum of essential nutrients, we end up swallowing a handful of pills. Sound familiar? The latest advertising images of various vitamins are shown as brightly colored blocks with human bodies jostling each other to try and rank importance. What insulting balderdash, in my opinion.

But how much do we truly understand what constitutes a vitamin ?

Here‘s an interesting exercise…

Can you draw the elements of a vitamin (in the same way that you can draw the descriptive details of a cat, a dog, an apple or any other basic concrete object)

I’ve asked this question many times and am not surprised by the responses. In truth, a couple years ago, I would have responded in like fashion with a simple sketch of a pill or capsule form speckled with dots to show different vitamins.

A vitamin is not a dot, or speckle, a pill, a habit or a label.

A vitamin cannot be manufactured by the body and must be obtained from our diet (or supplements). A vitamin is a biochemical catalyst for growth, maintenance and reproduction of all cells in the body. It is an integral part of hormones, genes, amino acids, minerals, enzymes and immune components.

We need vitamins and minerals to help fill in the gaps in our modern food supply to benefit from total nutrition and who doesn’t want that. Your choice of dietary supplement is one of the most important choices you can make for long term health for you and your family. The question is which kind of supplement? We better get the right source and validation.

Undoubtedly, the most important drawing you’ll ever do is to see the real components of a vitamin. So take a few minutes now and draw what you understand to be a vitamin. Don’t worry about different kinds (B, C, D, A, etc.) because they all have three common features.

Don’t read ahead until you have actually drawn something because you’ll be amazed at the picture of what constitutes a vitamin. It is the revelation that transfigured my perception of what Nature’s provision is all about.

So what are the features of a vitamin?

A vitamin is an organic compound composed of three elements: carbon, hydrogen and oxygen.

Different vitamins have different combinations of carbon, hydrogen and oxygen, some more a complex than others.

A drawing of a vitamin is a series of Carbon atoms linked with Hydrogen atoms linked with Oxygen atoms…all interconnected. For example, Vitamin C (ascorbic acid) shows a fixed matrix with 2 Carbon molecules,

7 Hydrogen molecules, and 6 Oxygen molecules

You can check out the actual configurations for different vitamins by google searching ‘vitamin structures.’ Some structures are impressively complex which further asks how best to replicate them or keep them as natural forms found in plants already.

Furthermore, an antioxidant is also a series of carbon, hydrogen and oxygen molecules…

A phytonutrient, or natural phytochemical, found in plants is a set of carbon, hydrogen, and oxygen molecules…

An amino protein is a combination of carbon, hydrogen and oxygen with a Nitrogen element…

A fatty acid is a long chain combination of carbon, hydrogen and oxygen.

Our natural whole plants are also composed of carbon, hydrogen, and oxygen molecules with the extra elements of minerals drawn from the soil. Make sure your mineral supplements also come from plants, not ground up rock or metals that can actually accumulate in the body’s tissues.

A natural question is to find out how many different configurations are found in plant foods. The answer is a second revelation. Fruits and vegetables contain hundreds of nutrients, vitamins, minerals, trace minerals, anti-oxidants, phytochemicals, enzymes, other co -factors and fiber. For example, if an apple is “labeled” correctly there would be over 600 nutritional elements that scientists can identify. This personal revelation finally led to a crossroads…how to best supplement…increase our portions of whole foods, supplement with whole food extracts, or take manufactured pills with “fractions” of natural plants.

In making the decision, a few principles stand strong. First, the body functions properly with proper “fuel” (food) a synergy of at least 100 nutrients. Second, the body is designed to draw nutrients from a natural food supply all working synergistically. Third, be aware that our genetic paleolithic blueprint hasn’t changed and that for thousands of years “ethnobotany” was the basis for healing. Fourth, was the choice to focus preventatively on natural whole foods to treat any prevailing unhealthy condition, rather than on the disease itself hoping for a chemical cure.

I decided to put my faith and trust in omniscient Nature rather than sifting and sorting through a huge number of products made in laboratories on conveyor belts.
